Engaging and Empowering Individuals from Coverage to Care
At HHS, we continue to work with our partners to help more people access affordable, quality health care. We are also focused on transforming our health care system to deliver better care, smarter spending, and healthier people. An essential part of this effort is empowering individuals to directly engage with their care and take ownership of their health.
On the 6th anniversary of the ACA, we celebrate the historic gains in Americans’ access to health insurance coverage – 20 million and counting! But our work isn’t over. We have an opportunity to help the millions of newly insured better understand their coverage and connect to their care in the right ways.
That means not only helping people pay their premiums, but to help them better understand their coverage, like what their cost sharing is, and how to connect to preventive services and other appropriate care – when and where they need it.

From Coverage to Care (C2C)
C2C offers resources at no cost designed to help consumers learn how to navigate their health care, understand their benefits, select a primary health care provider, and access preventive and chronic care management services.
- C2C’s Roadmap to Better Care and a Healthier You and other consumer tools are available in multiple languages at go.cms.gov/c2c.
- C2C is launching a web-based Partnership Toolkit in early April 2016, which will include ways to collaborate, ideas for events, print/online resources, prepared blogs and social media posts along with a new resource: 5 Ways to Make the Most of Your Health Coverage.
healthfinder.gov and myhealthfinder
- www.healthfinder.gov is a free resource that allows individuals to access reliable, easy-to-understand prevention and wellness information.
- www.healthfinder.gov/myhealthfinder is a free interactive tool that offers customized information based on the user’s age, sex, and pregnancy status about recommended preventive services available through most insurers without cost sharing.

- Develop Year-Round Programs for In-Person Assistance - HHS is interested in learning how you are building successful programs to assist individuals in your communities year-round, from enrolling in coverage to navigating the health care system.
